1. The application form prescribed by the government should reach the Chief Medical Officer, Chittagong EPZ Hospital, South Halisahar, Chittagong EPZ by post by 30/04/2014. No application will be accepted directly/hand to hand in this office. Applications received after the mentioned date will be considered cancelled. Application form can be downloaded from Bepza website: www.bepza.gov.bd.

Affidavit is not admissible for proof of age.
3. Incomplete, defective and late applications will be treated as rejected without assigning any reason. After preliminary selection only the most suitable candidates will be notified by letter to appear in written or oral or both tests as per the discretion of the authority.
4. Name of the post applied for and own district should be mentioned on the application envelope.
5. 600/- (Six Hundred) for the post of Medical Officer and Rs.500/- (Five Hundred) for the post of Senior Staff Nurse in favor of Chittagong EPZ Hospital, South Halishahar, Chittagong should be enclosed with the application.
6. The following documents should be submitted along with the application:
(a) Certificates of all educational qualifications attested by a Gazetted Officer of 1st Class.
(b) 03 (three) passport size copies attested by a Class 1 gazetted officer.
) Character Certificate issued by First Class Gazetted Officer.
Copy of National Identity Card attested by Class 1 Gazetted Officer.
(e) Attested copy of experience certificate (if any).
7. The authority reserves the right to accept or reject any application without assigning any reason and the authority shall not be bound to grant appointment or issue interview card due to the publication of this recruitment notice. Documents submitted with the application will not be returned.
8. Any form of advocacy or recommendation will be considered disqualification of the candidate. Candidates will not be given any TA/DA for appearing in written/oral test.
