Ministry of Health and Family Welfare, Department of Health Services Memorandum No- 45.00,0000 180,31,006, 23.2990 Dated- 01 January 2024 In accordance with the clearance received in the original, the Office of the Civil Surgeon, Noakhali in the various health institutions under the control of the revenue sector (grades 11-20). In order to recruit manpower for the following vacant posts, applications are being invited online http://csnoakhali.teletalk.com.bd from the residents of Noakhali district (natural citizens of Bangladesh) under the following conditions.

Application Conditions and Guidelines:-

Educational qualification and experience

a) Bachelor's degree in Statistics, Mathematics, Economics or equivalent from a recognized university.

b) Must have computer skills. Passed Higher Secondary Certificate or equivalent examination from a recognized board.

1. (a) Interested candidates can apply online as per the instructions published on the website at http://csnoakhali.teletalk.com.bd. No direct/postal applications will be entertained. The signature of the candidate must be identical in the application form, written, oral and practical (where applicable) examinations. Candidates who have applied in the 2018 recruitment circular do not need to apply again.

2. The latest regulations of the government will be followed in the matter of recruitment and quota determination.

3. Applicant age should be minimum 18 (Eighteen) and maximum 30 (Thirty) years as on 31/03/2014. However, the maximum age is 32 (thirty two) years in case of sons and daughters of brave freedom fighters/martyred freedom fighters and physically challenged. Affidavit is admissible for proof of age. Age will be proved on the basis of date of birth given in SSC Certificate/National Identity Card/Birth Registration Certificate.

4. Candidates employed in Govt/Semi-Government/Semi-Autonomous Organizations must apply within the stipulated time after obtaining the approval of the appropriate authority following the Government Rules (within the prescribed bus limits) and no objection letter issued by the appointing authority at the time of oral examination of all the post candidates ( NOC)- Cauliflower must be submitted compulsorily.

5. If the permanent address mentioned by the candidate in the application form is different from the permanent address mentioned in any certificate or elsewhere or if the husband's address is used in the case of female candidates, then in that case the candidate should submit the National Identity Card (NID) of the concerned municipality in favor of the changed permanent address. Citizenship certificate signed by Mayor/Councillor/Union Parishad Chairman should be submitted. In case of permanent address, if the ward number is wrong, the application will be considered as invalid.

6. For the post of Health Assistant, the candidate should be a permanent resident of the Ward (Old) for which he is applying. Permanent residents of other wards of the same union can also apply. However, if eligible candidates are not available from the residents of the concerned ward, eligible candidates from neighboring wards will be considered.

7. The list of vacant posts of Health Assistant (Ward, Union, Upazila wise) is available in the notice board and website (http://cs.noakhali.gov.bd) of the office of the Civil Surgeon and the office of the Upazila Health and Family Planning Officer.

8. If any information provided by the candidate is proved to be untrue or false or if necessary information is concealed or if any ineligibility is detected or if any fraud or corruption is resorted to or if he fills the form multiple times with false, different incorrect information and obtains multiple admit cards or if he takes unfair advantage of the examination or Misconduct at the examination center will lead to legal action against the candidate including cancellation of candidature or appointment at any stage before or after the examination or even after recruitment.

1. The candidate must be a citizen of Bangladesh by birth and a permanent resident of Noakhali District. If a candidate is not a citizen of Bangladesh or is married or committed to marry a person who is not a citizen of Bangladesh or has been convicted by a criminal court on charges of moral turpitude or has been dismissed from the service of any government or autonomous institution or local authority, he/she shall not apply. 10. Candidates can apply for only one post. If applying against more than one post, all his applications will be considered cancelled.

11. The application deadline and terms/conditions are as follows:-

(a) Online application form filling and examination fee submission start date and time 01/04/2014 from 10.00 am.

(d) Last date and time for submission of online application is 28/04/2014 up to 05.00 PM.

(c) Candidates who have received the User ID within the said period can submit the examination fee through SMS within the next 72 (seventy two) hours from the time of submitting the application online.

12. Candidates have to scan their color photograph (length 300 × width 300 pixels) and signature (length 300 × width 80 pixels) in the online application form and upload it at the designated place. Image size max 100 kb and signature sa
